Grants submission
Below you will find the forms most commonly used by Principal Investigators and collaborators with Meharry Medical College. Additionally, rights-only forms may be accessed via log-in to Meharry’s intranet system.
The forms are either Word or Excel documents with Meharry formulae/percentages already configured for you or PDFs upon which you can make direct entries. Either the Word/Excel file or the PDF form should be saved in your files with a unique name for your project. If for some reason you are unable to complete the form on-screen, you may download the form (that is, save the document to your computer), enter the information and send it to the Office for Research as an email attachment: NIH Forms, Including PHS 398, PHS 2590, PHS 416-1.

Many projects will require approval and oversight by one of Meharry’s institutional committees. Information about obtaining approval from the Institutional Review Board (human subjects research), Institutional Animal Care and Use Committee (animal research), or Institutional Biosafety Committee (hazardous and infectious material) is available through the OfRI’s Regulatory Affairs Unit.
Grant submission forms procedures
Under the new Meharry application submission guidelines, the PI and chair sign off on Grants Submission form (GSF). See below. The PI, chair, or the departmental business administrator forwards a copy of the signed GSF, budget, budget justification, and specific aims to the dean, and provost for review, approval and sign off. The Grants Management Office is copied during this pre-application phase of the process and expects to receive the complete application package at least ten business days before the funding sponsor’s deadline.

Grants submission timeline
Notify us as soon as you decide to apply for a grant, even if you haven’t started writing the application. We need your final documents two weeks before the submission date.
If Meharry is submitting the application:
1. Send your collaborator a Letter of Intent to Collaborate well before the deadline and ask him or her to obtain an institutional signature on it.
2. Provide these documents to Meharry’s Grants Management Office two weeks before the deadline:
- A budget for your collaborator’s school typed on an Excel facsimile of a PHS 398 budget page.
- A budget justification for your collaborator’s school
- Complete the Senior Key Peron Profile and return it to the GMO to save time in preparing the application.
- For NIH applications with multiple PIs, a Multiple PI/PD Leadership Plan (guidance can be found on this NIH page).
3. Provide these documents to Meharry’s Grants Management Office ten business days before the sponsor’s deadline:
- A Letter of Intent to Collaborate or similar document signed by an institutional official at your collaborator’s school. Collaborative applications will not be submitted by Meharry without this signed approval from your collaborator’s university.
- A Grant Submission Form signed by you (PI), your chair, the Dean, and Provost.
4. The PI should send a copy of the complete grant application after the application is submitted to the sponsor.
When all is in order, the application will be signed and submitted through grants.gov or according to the sponsor’s instructions.
If your collaborator’s school is submitting the application:
1. Provide these documents to Meharry’s Grants Management Office 10 working days before the deadline.
- The Specific Aims of the grant application, describing your role in the project.
- A budget for your part in the project, including Meharry’s F & A costs, typed on an Excel facsimile of a PHS 398 budget page
- A budget justification, including a full description of your role in the project.
- Complete the Senior Key Person Profile and send it to your collaborator to save time completing the application.
For NIH applications with multiple PIs, a Multiple PI/PD Leadership Plan (guidance can be found on this NIH page).
- A Grant Submission Form signed by you (PI), your chair, the Dean, and Provost.
- A Letter of Intent (sub recipient) or similar document furnished by your collaborator’s school.
2. When all is in order, the Grants Management Office will return the signed Letter of Intent to you.
3. Send the Letter of Intent to your collaborator.
4. After your collaborator’s school submits the application, provide the Grants Management Office a complete paper copy of the application.
