Health Assessments and Rapid Transformation (HEART)

Five years, three cities, and one mission to address cardiovascular disease

The School of Global Health at Meharry Medical College, in partnership with Novartis, has launched the Health Assessments and Rapid Transformation (HEART) Initiative—a five-year effort to improve heart health in three U.S. cities.

HEART takes a population health approach, addressing both clinical and non-clinical determinants of health. Guided by community voices and powered by research, policy, and technology, the initiative seeks to reduce cardiovascular health disparities and create scalable models that can be applied nationwide.

The HEART Initiative will focus its work in three geographically diverse U.S. cities—Nashville, Tennessee; Detroit, Michigan; and San Bernardino, California—each facing disproportionately high rates of cardiovascular disease and social vulnerability.
In collaboration with community stakeholders, including residents, health systems, nonprofits, and local governments, the Meharry School of Global Health will co-create, implement, and evaluate innovative clinical and non-clinical interventions that address the key drivers of adverse cardiovascular outcomes.
